Click on the '>' icon to access the 'Update item' page. Alternatively, click the red '+' for quick access to reporting issues.

Under Issues, click '+ Add new'.

You may also provide any general comments in the 'Remarks'. These are independent of the reported issue.

Provide details on the issue to be reported. Click 'Save' to officially report the issue.

The dropdown options for the following questions are customised by Agency admins when the checklist is created:

  • "What is wrong with this item?"

  • "How is this issue being resolved?"

If you have feedback about this, please contact your Agency admins.

You may add up to five photos (.jpeg or .png) per reported issue.

Report more issues by clicking '+ Add new'. Click 'Save' when you're done.

The question or item is now tagged with an issue.

Click on the 'Issues' tab on the navigation bar to view all issues reported in this page.
